Customized Community Supports (CCS) are designed to assist a person to increase independence, establish or strengthen interpersonal relationships, join social networks, participate in typical community life, and potentially reduce the amount of paid supports. CCS-I services are based upon the preferences and choices of the individual and designed to measure progress toward the Desired Outcomes specified in the ISP. Activities may include adaptive skill development, adult educational supports, citizenship skills, communication and social skills, self-advocacy, informed choice, community integration, and relationship building.
